Achieving the best possible outcomes for our patients has always been of paramount importance to cardiothoracic surgeons.  The process whereby we have learned to improve outcomes has been accomplished historically in large part through frank and honest review of our own performance.  This has included single institution centered mortality and morbidity conferences as well as less formal discussions among surgeons of near misses in which an adverse outcome was avoided by anticipation, action, or reaction on the part of the surgeon or surgical team member. 

This site is intended to leverage the global reach of the CTSNet cardiothoracic surgical community to amplify this process which has, heretofore, been largely restricted to individual practices and institutions.  The Cardiothoracic Surgery Safety Reporting System is intended to parallel the aviation safety reporting system which has been published on-line at http://asrs.arc.nasa.gov/.  Our goal is to create an archive of case vignettes submitted by the members of CTSNet describing potential errors or events occurring around the care of adult and congenital cardiac surgical patients and general thoracic surgical patients.  Each vignette will be presented anonymously and will include a description of the event as submitted by the individual.  The identity of the submitter is blinded to CTSNet. Vignettes will be reviewed and commented upon by a professional health safety expert. After posting by the Senior Editor they can be accessed through the EVENT REPORTS page. The eligibility to submit or review vignettes posted on CTSNet is restricted to registered CTSNet members.
